Form 1 and Forms 2 and 2a
Because the Church has continuous live streaming of the sanctuary, these forms are asking for your consent for your image to be streamed and transmitted via the internet as you undertake your role/s on the sanctuary.
Form 3 is to ensure that the parish holds relevant details for you.
Please would the following volunteers download, complete and return the relevant form to the safeguarding representative Anne Ryder at sg.staugustine.solihull@rcaob.org.uk or drop into the sacristy.
CONSENT FORMS
Form 1:
Adult Altar Servers, Cleaners, Eucharistic Ministers, Flower Arrangers and Readers
Forms 2 and 2a:
Altar Servers up to 16 years of age
- If the altar server is of an age to understand what they are agreeing to, please ask them to sign Form 2. Parents/carers must also sign form 2a.
- If the altar servers are too young to understand what they are agreeing to only parents/carers need to sign form 2a
- Altar servers aged 16-18 can sign form 2 and do not need parents/carers to sign.

Form 3:
All volunteers – Altar Servers, Children’s Liturgy, Cleaners, Counters, Eucharistic Ministers, Flower Arrangers, Gardeners, Organists, Readers, Stewards, Tea and Coffee Providers, Toddlers’ Group and anyone who volunteers for church duties.
